Request FSS Continuing Education (CE) Credits Letter

Use this form to request your FSS CE CREDITS LETTER certifying completion of an FSS workshop or training program.

Please use a separate form for each workshop or training session completed.

Once you submit the form, you will receive a confirmation and an email containing a link to download the "FSS Continuing Education (CE) Credits Support Documentation" describing the quality, authenticity, and efficacy of FSS training programs.

Within a few weeks you will be emailed your FSS CE CREDITS LETTER certifying completion of the FSS workshop or training program and the number of CE credits-contact hours earned. Please allow at least two weeks for processing your request.
