Verification of FSS Continuing Education (CE) Credits

STUDENTS: This form is for your LICENSING BOARD to complete, so they may independently verify that you completed an FSS workshop or training. Please click here to request your FSS CE credits letter.

LICENSING AND APPROVAL BOARDS-ORGANIZATIONS-ASSOCIATIONS: Complete this form to receive verification of a requesting FSS student's completion of workshops and training programs and CE credits earned.

Within a few weeks you will be emailed verification of the FSS student's completion of training and confirmation of FSS CE contact hours/credits earned. Please allow at least two weeks for processing your request.
